| Abstract: | How soon does irreversible nerve damage occur in central nervous system decompression sickness (CNS DCS)? To determine this we dove 27 pigs in a dry chamber using a profile that has reliably produced clinical decompression sickness in protocols previously reported. Nine of the animals, as controls, received no treatment. Nine of the animals were observed on the surface for 30 minutes and subsequently treated, while the remaining nine animals were observed for only 10 minutes before being recompressed. Treatment for the 18 animals was recompression to 100 FSW on air and then decompressed back to sea level over 200 minutes. Following treatment the animals underwent complete post mortem examination. Evaluation of sections of the brain and spinal cord revealed hemorrhages and gas bubbles in all three groups of animals. The occurrence of hemorrhages or Wallerian degeneration observed in any section of the CNS tissue was taken as evidence of nerve injury. 78% of the control group animals had hemorrhages. The 10 minute surface interval (S1) group had 3 animals (33%) with injury and the 30 minute S1 group had 2 animals (22%) with injury. Since all treated animals improved clinically, we conclude that the immediate recompression on air was effective in ameliorating CNS DCS. However, Wallerian degeneration seen in animals treated as soon as 10 minutes indicates that irreversible injury occurs within a very short period of time. |
