Abstract: INTRODUCTION: Each diver who reaches a deep depth for a long time and then reaches a saturation state, has to comply with decompression tables or computer indications to execute decompression in water stops and slow ascent. Despite the schedule has been rigorously observed, several divers suffer from decompression sickness. These pathological events are sometimes unexplained because of non valuable information about the diving circumstances. MATERIEL AND METHOD: During one year in 1999, in Reunion island, a census was made of all divers who suffer from an accident during diving or just after surfacing. Information has been collected from the records which contain some diving parameters and the explanations given by the participants. All divers used compressed air only. RESULTS: 19 cases were examinated. There were 15 males and 4 females aged from 26 to 60 years. 12 suffered from decompression sickness; 9 cases were well explained; 3 cases were unexplained: The depths were respectively 25 m, 10 m, 17 m for an undersea time of 22 min, 15 min and 35 min. The slow ascent was respected, a systematic in water decompression stop at 3 m for 3 mn was effectuated. No apparent cause may explain the occurrence of decompression sickness. However the information obtained from the record is incomplete and non valuable. It is then impossible to understand the reasons of the occurrence of the accident. CONCLUSION: The purpose of this paper is to propose the implementation of a register collecting all the facts of the accident without trying to blame anybody. It could be expected that anonymous explanations would be more exact and really effective for comprehension and prevention of decompression sickness. The standardized data collection could be divided in 3 parts: the victim, the diving accident, the medical care. decompression illness, database, incident record,
