| Abstract: | MATERIALS AND METHODS: Audit forms were returned to Aberdeen for checking against the patient record and for analysis. Cases were categorised in terms of severity. Mild to moderate disease included pain only, sensory or ataxic presentations. Severe illness included motor weakness, nausea and vomiting and cerebral presentations. Time to treatment, response to initial recompression and condition on discharge were analysed for patients flown to Aberdeen for treatment and patients treated locally. RESULTS: The table indicates severity and response to treatment. Median time to treatment was significantly shorter for Orkney treated patients (2.5 hours) than in Aberdeen treated (6.1 hours). Complete response to initial recompression was more frequent in Orkney treated cases. There was little difference in final outcome, however, although more cases with mild to moderate disease went home with no sequelae from Orkney (87percent) than from Aberdeen (66percent). One case in the Aberdeen series died during treatment. One moribund case was unsuitable for treatment in Orkney and was evacuated to Aberdeen for successful treatment. CONCLUSIONS: The operation of a local recompression unit in Orkney has improved initial treatment results and the outcome of mild to moderate disease. Although initial treatment results for severe disease were also improved there was no evidence for improved outcome in this group |
