| Abstract: | Seven patients with non-healing, ischemic lower extremity wounds without osteomyelitis were evaluated with PET using (18)F-deoxyglucose (FDG). Revascularization was deemed impossible in all patients. Scans were obtained simultaneously of both legs. An average of 7 images (range 3 to 9) were obtained on each patient after injection of an average of 10.1 millicuries (mCi) of FDG using a ECAT III scanner (CTI, Knoxville, TN). Following the PET study 5 of 7 patients underwent hyperbaric oxygen (HBO) therapy at 2 ATA for 2 hours. The average number of HBO treatments was 25.6 (range 12 to 54). Of the patients treated with HBO one died of other causes within a month, three required BKA, one underwent skin grafting and healed successfully after a second course of HBO. The two patients who did not receive HBO healed eventually with successful skin grafts. Increased FDG uptake was consistently noted in the affected limb, presumably indicating hypermetabolism. There were no obvious differences between healers and non-healers in the pattern of FDG uptake. One patient had increased uptake in the contralateral limb and eventually required amputation on that side. This preliminary study suggests that metabolism in areas adjacent to non-healing wounds is higher than in normal tissue, despite clinical evidence of ischemia. Evidence of increased FDG uptake is not necessarily a sign of viable tissue. |
