| Abstract: | As part of a baseline wound oxygen (O2) evaluation, patients presenting for hyperbaric oxygen (HBO) therapy were assessed by transcutaneous oximetry (Radiometer TCM2) to determine normobaric Ptc02 responses to 0.21 atmospheres absolute (atm abs, 21 kPa O2) and 1.0 atm abs inspired 02 (101 kPa 02) and hyperbaric responses to 2.36 atrn abs inspired 02 (239 kPa 02). Ptc02 values were recorded from chest and wound sites every five minutes ever three 25-minute periods while patients first breathed air, then 100% O2 (normobaric and hyperbaric) via a clear vinyl hood (Sealong) in a hyperbaric chamber. Ptc02 data were analyzed according to severity classification and cutaneous characteristics of the wound. Tests of repeated - analysis of variance used to assess mean Ptc02 and calculated mean Ptc02 index (wound/chest) differences over time within each group and across treatments at each time increment. Linear and curvilinear regression techniques were used to predict mound Ptc02 values at 2.36 atm abs from normobaric wound Ptc02 and Ptc02 index values. Early observations suggest that wound Ptc02 and Ptc02 index responses to inspired air and 100% 02 at 1.0 atm abs are poor predidms of wound -2 and %dl2 index responses to 100% 02 at 2.36 atm abs. Wound Ptc02 and Ptc02 index values during HBO in some patients mere up to 12 times greater than mound Ptc02 values in those same patients during normobaric 100% 02 breathing periods. Pretreatment wand Ptc02 and Ptc02 index responses to HBO may be useful in determining the appropriateness of subsequent HBO therapy. |
