Abstract: To elucidate possible patient characteristics that may predispose to mortality during necrotizing infection, we reviewed the records of all patients referred for HBO therapy of necrotizing infection in the past 10 years - a total of 89 patients. Site distribution, type of infection and mortality were as follows (deaths in parentheses): <table> --, Trunk, Leg, Arm, Perineum, Head; Clostridial, 15 (2), 10 (3), 3 (2), 5 (0), 2 (0); Non-Clostridial, 20 (4), 18 (4), 6 (1), 7 (2), 0 (0); TOTAL, 35 (6), 28 (7), 9 (3), 12 (2), 2 (0) </table> The mean age of survivors was 49 years, and of patients who died was 58 years (p < 0.05). Statistically significant relationships to mortality were found for the following factors: TISS score, requirement for mechanical ventilation, diabetes mellitus, and renal dysfunction. There was no demonstrable relationship between mortality and the presence or absence of Closmdial infections (7 of 35 with Clostridia died, vs. 11 of 54 non-Clostridial), gender (10155 males died vs. 8/34 females), ethanol abuse, drug abuse, coronary artery disease, peripheral vascular disease, hypertension, malnutrition, neoplasm, obesity, or tobacco abuse. However, the sample size was too small to rule these out as prognostic factors. Overall mortality in our patients (20.2%) compares favorably with other published series.
