| Abstract: | Injury to the bladder is an unfortunate side effect of therapeutic radiation administered for a variety of pelvic malignancies. Radiation-induced cystitis is often a troublesome problem for both the patient and physician. Significant symptoms primarily include urinary urgency, pain, and recurrent hemorrhage. At the Virginia Mason Medical Center, 12 patients who had failed all other attempts at management and who had no evidence of infection or recurrent malignancy were treated with hyperbaric oxygen therapy for radiation-induced cystitis. Patients received 2.4 ATA oxygen for 90 minutes five to six times per week for an average of 20-30 treatments. During follow-up, ranging from six months to three years, five patients have experienced complete resolution of symptoms. Five other patients have noted marked improvement, one of whom stopped treatment early due to claustrophobia. One patient had minimal improvement due to recurrent malignancy and one patient was withdrawn after only nine treatments due to illness, dying later of sepsis. Average cost was $10,000 to $15,000, comparing favorably to the cost of multiple "conservative" treatments to control patient symptoms. We conclude that hyperbaric oxygen therapy for radiation induced cystitis is an efficacious modality in patients who fail other forms of management. Data from two additional patients will be added to this study for the presentation. |
