Abstract: BACKGROUND: Decompression incidents have been divided into 'deserved' and 'non-deserved' by some experts dependent upon whether or not the incident dive was in accordance with an accepted decompression table or dive computer algorithm. This study investigated the role of provocative dive profiles in initiating decompression illness. METHODS: The BHA/INM diving accident database was scrutinised to identify all cases of DCI treated by member chambers in the 6 year period 1995 to 2000. Analysis of all dives completed by the diver prior to presenting with symptoms of DCI was made to identify potentially provocative dive profiles. RESULTS: 883 cases of DCI were identified from the 1241 diving incidents reported to the BHA/INM database during the study period. In 264 (29.9percent) cases of DCI a rapid ascent was reported whilst in 124 (14percent) cases multilevel diving had been performed. An additional 12 cases (1.4percent) were identified in which there was both a rapid ascent and multilevel diving. Thus, in 400 cases (45.3percent) there was evidence of a provocative dive profile. In addition to the 883 cases of DCI 61 cases of omitted decompression were identified from the 1241 diving incident cases. CONCLUSIONS: In nearly a half of the cases of DCI treated within the UK between 1995 and 2000 analysis of the dive profiles reveals a provocative dive profile. A history of a rapid ascent appears to be particularly significant. It is of note that some divers, despite reporting feeling unwell following a rapid ascent, continued to dive until presenting with more severe and undeniable symptoms at a later date. It is strongly recommended that divers who experience a rapid ascent should avoid further dives for a number of days.
