dc.description.abstract A review of the literature suggests that patients with insulin dependent diabetes (IDDM) who are undergoing hyperbaric oxygen therapy may have increased glucose metabolism and more importantly the potential risk of hypoglycemia. As part of our Performance Improvement Process, we studied our hyperbaric patients with IDDM to validate the changes in blood glucose levels on all diabetic patients with IDDM to validate the changes in blood glucose that have been reported in the literature. We obtained pre and post-treatment blood glucose levels on all diabetic patients who underwent hyperbaric oxygen therapy, at our three hyperbaric centers. Our goal was to identify changes in blood glucose levels in our patients and use this data to establish threshold levels that would prompt intervention to prevent hypoglycemia. Methods: Data was analyzed on a total of 272 individual chamber patient/treatments at 1 multiplace and 2 mono place HBO treatment sites We obtained average declines for the entire patient population - and segregated the population into a grouping of patients who presented with levels of 180 or less (total of 129 patient/ treatments) the data breaks down as follows: Results: 272 total patients Average decline 62.7 mg/dl, 129 pts at 180 or less on presentation Average decline 37.3 mg/dl, All patients with a blood sugar of 160 or less received post treatment blood glucose. Those with pre treatment level less than 110mg/dl received in chamber glucose supplements. Conclusions: Analysis of our data suggested that appropriate interventions include treatment with glucose prior to hyperbaric therapy if blood glucose levels were less than 110. In addition a recheck of blood glucose level following hyperbaric therapy was indicated if pre treatment levels were less than 160. Implementation of this new protocol assures prompt treatment of patients with hypoglycemia and proper assessment of our diabetic patient population during their hyperbaric care. en_US
