Abstract: BACKGROUND: Some patients undergoing hyperbaric oxygen therapy have subjectively reported a decrease is vision during treatment. The purpose of this study was to determine the incidence and magnitude of refractive changes in patients receiving hyperbaric oxygen therapy. METHODS: 43 consecutive patients (86 eyes) were examined prospectively while undergoing hyperbaric oxygen therapy. Best corrected vision, cycloplegic refraction, and slit lamp examination were performed at pre and post therapy visits. RESULTS: Overall, there was a mean change of -0.64 +/- 0.11 (dioptres +/- sem) (p less than 0.0001). Subgroup analysis demonstrated that in the 75 eyes that were phakic, there was a mean change of 0.74 +/- 0.12 D (p less than 0.0001). In the 11 eyes that were pseudophakic, there was a mean change of 0.03 +/- 0.05 D (p = 0.66). CONCLUSIONS: Hyperbaric oxygen therapy causes a significant myopic shift, exclusively in phakic eyes, suggesting a lenticular etiology of refractive change. This study provides harder numbers on the predictability of the ocular effects, which hyperbaric patients commonly encounter.
