Abstract: BACKGROUND: Current recommendations for assessment of carbon monoxide (CO) poisoning include obtaining an arterial carboxyhemoglobin level (A_COHb). We tested the hypothesis that venous COHb levels (V_COHb) would predict A_COHb with sufficient accuracy that patients could be spared the discomfort ofarterial puncture. METHODS: Simultaneous arterial and venous COHb levels were obtained on an unselected convenience sample of patients with suspected CO poisoning. The study took place between 1991-1993 in a municipal ED with a contiguous hyperbaric (HBO) chamber. COHb levels were measured using a standard spectrophotometric assay. RESULTS: A_COHb levels in 66 patients ranged from 0.9% -39.3%. The median difference between A_COHb and V_COHb in each individual was 0.4% (interquartile range 0.2% -0.6%). Linear regression with V_COHb as the independent variable and A_COHb as the dependent variable revealed r = .99 (95% confidence interval {CI} [.99, .99]), r 2 = .98 for the entire dataset. For A_COHb < 20%, r =.98 [95% CI .97, .99], r2 = .97; however, for A_COHb 2: 20%, r = .90 [95% CI .54, .98], r 2 = .81. CONCLUSION: V_COHb levels with a high degree of accuracy, especially at levels below 20%. Patients with V_COHb < 20% can be safely spared an arterial puncture, and, if asymptomatic, do not require HBO treatment. Until more data are accumulated in patients with levels> 20%, arterial COHb are still recommended.
