Abstract: The decrease in tissue hypoxia obtained by hyperbaric oxygenation (HBO) counteracts the effects of reflex vasomotor disturbances caused by traumatism, in Sudeck's Syndrome. The increased tissue metabolism induced by traumatism determines local lack of oxygen in spite of the increase of vascularity and blood flow, with relative anaerobism and local acidosis; chronic edema results from venous overload and passive capillary repletion. The hypoxic state induces undifferentiated cells and proliferating fibroblast to a quick maturation making them pass to a state which requires a lower oxygen consumption. All this determines an abnormal production of fibrous tissue which causes retraction, adhesions and joint stiffness. Acidosis causes demineralisation and bone protein catabolism. In our experience, HBO proved to be very effective, even after a few treatments,to resolve local edema and to relieve pain in patients who had not positively reacted to other therapies. Scleroatrophic evolution has always been avoided. All the patients treated with HBO showed the typical radiographic features ( with acute, patchy osteoporosis ); the resolution of this picture, however, was always slow. The authors propose a staging of the disease, in order to choose proper treatment, and objective standards for results evaluation. Many clinical cases are presented.
