Abstract: BACKGROUND: A system was required to regulate and distribute the breathing gases for a randomized, double blind placebo controlled study. METHODS: The placebo gas, oxygen and BIBS (built in breathing system) gases were passed though pressure regulators to equalize pressures in order to eliminate flow variances when switching between gases. The individual gases were then piped to a gas distribution box. The distribution box had 12 selector valves which routed the selected gas to a specific seat position in the chamber. Any eight of the valves could be set up to route placebo or BIBS to the patient, the four remaining valves were not piped to any gas or seat position and served to help cover the blinded gas distribution. The valves were periodically reconfigured internally to change the valve assignments. Side valves on the box determined whether placebo and BIBS were routed through the box or if only BIBS was being delivered to the patients. The setup and configuration of the box was known ony to the two people responsible for randomizing the patients into the study. RESULTS: The box served to protect the randomized status of the patients in the study from the technicians, nurses and physicians while allowing non-study patients to be treated at the same time. CONCLUSIONS: The system could not be used with confidence in blinded studies, for multilevel exposures at a single ambient pressure and for non standard air breaks during standard treatments.
