Abstract: BACKGROUND: Avascular necrosis (AVN) of the humeral head is a common complication related to sickle cell disease. While the use of hyperbaric oxygen (HBO2) therapy in the treatment of AVN has been described in the hip, no description exists for shoulder lesions. The purpose of this case report is to detail use of HBO2 therapy as an adjunct in treatment of acute pain crisis as a result of bilateral humeral head AVN secondary to sickle cell disease. MATERIALS AND METHODS: We report on a 20 year old African American female diagnosed with sickle cell disease (hemoglobin SS). The patient developed bilateral shoulder pain while performing activities as a cheerleading coach requiring inpatient admission for acute pain management, resulting in initiation of high dose narcotics with poor pain control. Orthopaedic consultation was initiated after workup revealed bilateral humeral head AVN on MRI, who recommended a trial of HBO2 therapy to address the patient's symptoms. Her regimen included 10 treatments at 2.4 atmospheres absolute (ATA) for 90 minutes. RESULTS: The patient's acute pain was controlled after 2 treatments of HBO2 therapy. This resulted in discontinued use of narcotic pain medications substituted with only occasional NSAID treatment. In addition to pain control, repeat MRI of bilateral shoulders demonstrated decreased edema within the humeral heads, suggesting radiologic evidence of healing of the AVN lesions. CONCLUSIONS: HBO2 has been used in the treatment of femoral head necrosis with reported short-term improvement of 92% and long-term improvement of 81%. This case report describes HBO2 therapy used to treat acute pain secondary to humeral head AVN from sickle cell crisis. This case report suggests HBO2 therapy may be useful in the treatment of AVN in locations other than the femoral head.
